Your 1st month of parking is defined as the first 30 days from the Start Date.
your second month will be prorated to have your monthly billing cycle be the
first of the month.
CUSTOMER INFORMATION (ACCOUNT HOLDER)
GARAGE WILL ONLY RELEASE CUSTOMER'S VEHICLE TO THE PEOPLE WHOSE NAMES ARE PRINTED BELOW:
Garage will issue Customer a Spot # and/or an Account #, as well as an indentification sticker that will be placed on Customer's vehicle. Garage, NOT Customer, will
decide the location of such sticker on Customer's vehicle. This sticker is valid ONLY for the vehicle registered on the this form and is not transferrable.
Substitute/replacement vehicles may be accepted at the discretion of Garage but are in no way guaranteed to be permitted in the Facility without being charged
the transient rates of the Facility. Garage has the right to move Customer's vehicle to any of its affiliate locations for special circumstances; if this happens,
Customer will still be able to pick‐up and drop‐off their car at the Facility.
Monthly Space Account Activation:
By signing below, the undersigned (Customer) acknowledges and agrees to all terms and conditions on this Monthly Space Account Registration form, no
exceptions. By signing below, Customer is agreeing to rent Monthly Space from Garage at the Facility on a recurring monthly basis until their Monthly Space
Account is cancelled subject to the Cancellation Terms outlined on this form, no exceptions. By signing below, Customer is officially activating their Monthly Space
Account with Garage and hereby promises and agrees to pay Garage all parking charges (Parking Rent, account activation fees, etc) that accrue to their Monthly Space Account, no exceptions. By signing below, Customer certifies that all information they have given on this form is correct and true.
By signing below, the undersigned (Customer) hereby authorizes Garage to charge the credit card shown above (and/or a different card of Customer's already on file) for all parking charges (Parking Rent, account activation fees, etc) that Customer accrues to their Monthly Space Account. This authorization will continue in force and perpetuity until Customer's Monthly Space Account is cancelled subject to the Cancellation Terms outlined below, no exceptions.
Customer can only cancel their Monthly Space Account by sending written notice EXCLUSIVELY VIA EMAIL to firstname.lastname@example.org, no exceptions; verbal or written
cancellation with Garage is not acceptable. Cancellation must be given fifteen (15) days prior to the first day of your billing cycle. If 15 day noitice is after the first
day of the billing cycle the entire months fee or the daily rate will be charges, whichever is less. If Customer prepays Garage for a period of time greater than one
month as part of a prepaid multi‐month promotion (i.e. six month prepay), and during such period wishes to cancel their Monthly Space Account before such
period is completed, then Customer will not receive and is not entitled to any refund for the unused portion of such prepaid multi‐month promotion, no
exceptions. If Customer breaches any terms and conditions on this form, then Garage may cancel Customer's Monthlys Space Account immediately, no exceptions. Garage can cancel Customer's Monthly Space Account with 7 days notice for any reason, with the only exception being if Customer is engaged in a prepaid multi month promotion.
Miscellaneous Terms & Conditions:
Monthly Space is defined as any form of space/area(reserved space, a reserved area, leased space, a leased area, assigned valet and self parking spaces, unassigned
valet and self parking spaces, etc.) at the Facility that Customer is renting and/or licensing from Garage. Different types of Monthly Space are taxed differently.
Parking Rent is defined as the rent or license fee that Customer pays Garage for Monthly Space. Parking Rent for Monthly Space is not based on days used;
Monthly Space is rented on a calendar month basis only; specifically, the Monthly Space rental period is from the 1st of a given month thru the last day of that
month, with the ONLY exception being Customer's 1st month of parking (and associated 2nd month prorate period, if any); no offsets or refunds of any kind will be
given to Customer for the unused portion of any month they paid for. Parking Rent is always due in advance of use. Customer's Monthly Space Account balance
must be zero at 6am on the 1st of each month or it will be considered delinquent and will be subject to daily rates and a $50 late fee until Customer's Monthly
Space Account balance is is zero, no exceptions; additionally, Customers who have a delinquent Monthly Space Account may have their vehicle placed on HOLD by
Garage and unavailable to the them until all parking charges that are due to Garage have been paid in full and Customer has paid their Monthly Space Account
balance down to zero. Unless Garage, at its sole discretion, makes an exception and expressly allows Customer to pay parking charges (Parking Rent, account
activation fees, etc) that accrue to their Monthly Space Account by cash or check (with such exception evidenced in writing on the reverse side of this form next to
the Credit Card Authorization paragraph, and with such exception subject to being revoked by Garage for any reason at any time), Parking Rent and all other
parking charges that accrue to Customer's Monthly Space Account must be paid by Customer to Garage exclusvely via a credit card left on file with Garage. Parking
Rent is subject to increase at the discretion of Garage. Notice of such increase will be via display/signage in the Facility and/or an emailed rent statement and/or a
paper rent statement to be placed in/on Customer's vehicle. If a Parking Rent increase was already stipulated in a promotion that Customer is engaged in with
Garage, and/or entered the Facility under, then no secondary notice will be given to Customer by Garage. If Customer began renting Monthly Space at the Facility
as a result of a promotion and/or is given a temporary discount for their Parking Rent by Garage, then Customer must not discuss the terms of such promotion or
temporary discount with any other Customer of the Facility; if Customer does this, then Garage may immediately cancel such promotion or temporary discount
that Customer is receiving. Customer must notify Garage with written notice sent via email to email@example.com if their vehicle is going to change from the one
registered on the reverse side of this form; Customer's Parking Rent may immediately increase as a result of such change. Garage not responsible for Customer's
vehicle (as relates to each time Customer's is trying to drop‐off/leave their vehicle at the Facility) unless Customer turns their vehicle off AND delivers the key to
Garage staff AND Garage staff acknowledges acceptance of vehicle from Customer, no exceptions. Garage not responsible for removable equipment or personal
property (cash, jewelry, laptops, etc.) left in Customer's vehicle, no exceptions. Garage not responsible for damages to bumpers on Customer's vehicle of any kind
(nicks, scratches, dents, etc), no exceptions. Garage not responsible for damage claims to Customer's vehicle unless such claims are reported to Garage staff AND
specified in writing PRIOR to Customer's vehicle leaving the Facility with signatures by BOTH Customer and Garage staff. Customer holds Garage harmless against
all claims arising while Customer's vehicle is not in custody of Garage. Pursuant to law, Garage's liability (due to damage, theft, etc.) for Customer's vehicle is
limited to $25,000. Customer must adhere to all terms and conditions displayed on signs within the Facility, no exceptions.
